COPD Post Stabilization and Monitoring
Pathway Objectives
To detect exacerbation and support patient with an individualized action plan, which will assist with the patient remaining safe at home and preventing the need for an ER/Urgent care visit
Timeline: Daily, ongoing
Circle of Care – Patient & Care Team
Nurse, Physician, Respiratory Therapist
O2 reading is taken from their Bluetooth enabled device and inputted into the platform or patient is prompted to manually enter their O2 reading
Daily questionnaire monitoring patients symptoms including shortness of breath, feelings of distress or danger, and overall well-being
Symptoms are monitored and education is triggered based on patient responses
Patient specific interventions are provided on assessment and O2 readings i.e., breathing exercises, education resources, medications, rescue medications, and/or virtual contact with the care team
Education is provided in the form of resources and reminders/pictures i.e. images of reliever medications and techniques
Clinician interventions can include phone calls or video visits with the RT; for extreme symptoms of exacerbation or if patient feels they are in immediate danger, the patient is instructed to go to hospital ER
Based on patient responses and symptoms, reassessment can occur within 12, and 24 hours where clinician is monitoring and engaging with the patient as required
