Pathway Objectives
To assist patients with maintaining blood sugar levels within normal range and provide timely educational resources and medication reminders to ensure a healthy blood sugar range is maintained.
Timeline: Daily, in the morning and prior to meals
Circle of Care – Patient & Care Team
Nurse, Diabetes Educator
Prompts patient to check blood glucose levels at specific intervals during the day; values can be manually inputted or are obtained if their device is Bluetooth enabled
Based on the readings, appropriate corrective interventions are provided to the patient to encourage the patient to self-manage effectively at home i.e., have a snack, review care planning goals, take insulin, patients are notified to re-check blood glucose 2 hours post intervention
Patients are triaged based on their blood sugar readings, those that require immediate attention are contacted by the diabetes educator for a virtual visit
