Pathway Objectives
To enable specialized centres to provide virtual support to people (adult and pediatric patients) using home mechanical ventilation (HMV) to help
them feel connected and safe.
Timeline: Weekly. Ongoing. In-person visits every 3-6 months with HMV clinical team
Circle of Care – Patient & Family Caregiver & Care Team
HMV Clinical Team, Ventilator Support Coordinator, RN, RT, MD
Patient and caregiver are onboarded to include Respiratory Technology information, ventilator settings, cough assist settings, medications, medication coordination, health directives, care planning and home and community supports
Offers a multicomponent intervention that includes virtual home visits, customizable care plans and clinical workflows for respiratory symptoms and signs. It also addresses ventilator issues
The platform enables telemonitoring of oximeters and ventilators, secure communication via messaging, audio, and video calls among patients, families, and health care providers.
This program includes :
-Teleconsultation with families and patients by the HMV team to assist with symptom management
and vent troubleshooting
-Telemonitoring with Bluetooth transmission of vent data downloads accessible at any time by the
HMV team
-Teletherapy which includes consultation with the care team to lead and assist families with
prescribed vent settings and changes
