Pre/Post Cancer Surgery
Pathway Objectives
To monitor patients pre and post-op to optimize Cancer care outcomes
Timeline: Two days prior to the scheduled surgery until post-op day 30
Circle of Care – Patient & Care Team
Nurse, physician, service provider staff
Prior to surgery patient is enrolled and initial assessment is conducted including vitals a sociodemographic assessment, and functional assessment; subsequent reassessments are scheduled based on patient needs
Real-time patient generated health data through post op vitals monitoring (pulse oximetry, temperature, blood pressure, heart rate, and weight assessed); the care team is notified of any measures out of normal threshold
Clinician interventions can include a planned phone conversation, a clinic visit scheduled next business day, or urgent care/ER visit
Monitors if patient is readmitted to hospital and continues to monitor patient if readmitted (all questionnaires continue), this offers an integrated care between community and team in hospital teams (if admitted)
Electronic patient-reported outcomes through questionnaires are provided (POD2, 7, 14, 30) to assess symptoms, mobility, activities of daily living, pain/discomfort, depression, and quality of life post op; patient responses that are out of range are triaged and care team is notified
