Pathway Objectives

To provide the necessary support to COVID-19 positive patients isolating at home. Allows patients to feel safe that someone is monitoring their care at home.

Timeline: Daily for 15 days.

Circle of Care – Patient & Nurse

Daily questionnaire that begins on Day 1 that reviews their ability to perform activities of daily living over the last 3 days independently

Asks patients if they have support with accessing i.e., groceries, medications, necessities during isolation period

Monitors symptoms including vitals and if they are worsening

Patient is prompted to perform questionnaire daily for 14 days

Patient is triaged based on whether they respond to the daily questionnaire and if they lack support in the home; RN is notified if patient symptoms worsen and if patient lacks in-home support during their period of isolation (a virtual visit is then scheduled)

At day 15 patient is discharged from program
