Pathway Objectives

To support the safe self-management of seniors 70yrs+ who have had cardiac surgery and are being discharged home.

Timeline: Daily, for four months

Circle of Care – Patient & Care Team

Nurse, Physician

Monitors patient’s vitals (manually input or Bluetooth enabled) and daily wound assessment

Patients are triaged for clinician intervention based on their responses that fall out of normal range

Patient is guided through questionnaires that assess BP, weight gain/loss, blood glucose, oxygen levels, pain levels, and appearance of their wound (via uploaded photos)

Patient has access to the Surgical Nursing  Coordinator if they feel unwell or have been alerted to contact their nurse based on their  responses to the questionnaire
