Pathway Objectives

Allows patient to manage their pain-level and medications safely at home. The care team has real-time access to pain assessments and dose trending to assist with titrating pain medications immediately.

Circle of Care – Patient & Care Team

Offers the patient with the ability to take ESAS palliative assessment that monitors the common nine symptoms in palliative patients (pain, tiredness drowsiness, nausea, appetite, depression, anxiety, shortness of breath, and wellbeing) – weekly

Daily pain diary is also provided to the patient

Patient is prompted to take a daily pain assessment and provide info on meds taken and how many doses taken during last 24 hrs

All pain medication information including doses (includes breakthrough medications also accessible to the
patient) are monitored as shared with the care team

Care team can add thresholds on doses and are notified if number of doses taken are beyond the set threshold
